Traditionally, health management organizations have mandated that primary care doctors address issues before referring patients to specialists. Carrum, however, has adopted a different strategy, prioritizing negotiations for high-quality care at lower prices. While headquartered in San Francisco, Jain’s connection to San Diego is profound; he moved to the city with his family in 2023, after spending a decade in the Bay Area where Carrum was founded.
Building a Foundation in San Diego
Jain recalls the significance of San Diego in Carrum’s history, particularly its first partnership with Scripps Health. He was introduced to Scripps CEO Chris VanGorder, and they worked together to create a model that would challenge the status quo in healthcare delivery. Jain noted, “Our very first center of excellence provider was Scripps Health, and they saw the potential in what we were trying to achieve.”
During the initial years, Jain frequently traveled between San Diego and the Bay Area to refine their approach. “Every time I’d come down here, my heart rate goes down. It was something about the weather and the people,” he remarked. This connection to the region has influenced Carrum’s operations significantly.
With an academic background in computer science and an MBA from the University of Chicago Booth School of Business, Jain’s journey into healthcare began in 2009 when he entered the hospital revenue cycle sector. His experiences revealed inefficiencies within the system, particularly in how patients navigated care and billing. He expressed concern about the lack of transparency for patients regarding their healthcare options.
“Patients have no idea who the good doctors are,” Jain explained. “When I saw how it worked, I was like, ‘this is just mind-bogglingly bad.’” This realization motivated Jain to establish Carrum Health with the aim of improving outcomes while reducing costs.
Data-Driven Solutions
To tackle inefficiencies, Carrum Health employs a data-driven approach. Jain explained how the company utilizes vast amounts of data, including clinical and claims information, to identify high-quality providers. “We sift through terabytes of data and look for signals of providers who are doing good quality work,” he said. Once identified, Carrum engages these providers and requests comprehensive data on various performance metrics.
The company also implements a risk-sharing model with top-tier doctors and health systems, which incentivizes them to ensure positive outcomes. If complications arise from a procedure, the providers are responsible for associated costs. “This structure encourages them to get it right the first time,” Jain added. Typically, Carrum can negotiate prices that are about half of what employers would normally pay.
Addressing Substance Use Disorders
In a significant expansion, Carrum has recently focused on substance use disorders, a critical area given that approximately one in six Americans struggles with addiction. Jain emphasized that employers cannot afford to overlook such a substantial portion of their workforce. The stigma surrounding substance use often prevents employees from seeking help locally, making it imperative for companies to provide accessible treatment options.
“Instead of paying per visit, we pay for the entire episode of care,” Jain explained. “If a patient relapses, we penalize the provider.” This innovative approach aims to enhance treatment effectiveness while managing costs for employers.
Jain’s upbringing in India, where healthcare operates on a cash-pay basis, has also shaped his perspective on the U.S. healthcare system. He noted, “In India, there’s a direct interaction between the patient and the provider, which eliminates many middlemen.” This experience has reinforced Jain’s commitment to creating a more efficient and transparent healthcare model in America.
